New Local NHS Programme
Our practice is taking part in a local NHS programme to help understand the health needs of our patient population.
As part of this, a system called the Johns Hopkins Risk Stratification tool has been run in our clinical system. This tool helps NHS teams plan services and resources by grouping patients based on factors like age, existing health conditions, and how often they use healthcare services.
These groups are called Patient Need Groups (PNGs) and range from 1 to 11, with higher numbers reflecting greater overall health needs.
What does this mean for you?
- The PNG is generated automatically based on your health record.
- It does not change your current treatment or care.
- It is used for planning at a population level — not to make decisions about individual patients.
You may see your PNG listed in the NHS App. Please don’t worry — it does not represent a diagnosis or any change to your care.
You do not need to contact the practice about this.
If your care needs change in the future, we will always discuss this with you directly as usual.
If you have an existing condition or care plan, it remains unchanged.
