How to Register

You are very welcome to register with the practice if you live within the practice area as shown below.

Catchment Area

To register with us simply complete the form via the link below, if you are unable to complete this online please print the PRF1 Form and bring to Park Surgery. Our Administration team will contact you if there are any queries with your registration within two weeks of submitting your registration.

Non-urgent advice: Please Note:

If you are outside of the practice catchment area you may have the option to register as an ‘out of area patient’ providing certain criteria is met. Each application to register ‘as an out of area’  must be reviewed prior to approval. If you register as an ‘out of area’ patient you should understand that you will be unable to have home visits from a GP at this surgery and it is important to note that certain health needs will need you to visit the practice in person and telephone consultations may not be suitable.

New Patient Information

Please ensure you have enough medication from your previous practice to cover you for a few weeks as it takes some time for your information to move over to your new practice.

Regarding your medication, please email a copy of your repeat slip to our Admin team kmicb.pm.parksurgery@nhs.net, alternatively please drop it in to Park Surgery.
