Warfarin monitoring

The practice nurses run anti coagulation clinics on Tuesday morning at Herne and Broomfield Surgery, and on Thursday morning at Park Surgery. The warfarin clinics are run by Nurse Rebecca Lock, Nurse Justine Baker, Nurse Justine Jones and Nurse Theresa Alcock. When attending your warfarin clinic appointment please bring your yellow book and any further information you may have.

Home visits for warfarin monitoring are available for housebound patients only. These visits are generally on a Tuesday morning.

Prior to your travel vaccination appointment please complete a travel risk assessment form and return it to the surgery. These can be collected from reception or alternatively you can download one using the link below.

Respiratory

Respiratory clinics are run by Nurse Justine Baker, Nurse Theresa Alcock and Nurse Emily Lock. Appointments are available at both The Park and Herne & Broomfield Surgery.

When attending a respiratory clinic appointment please bring all inhalers with you, if you have any. Please also print and complete the below ‘Asthma Control Test’ document and bring it with you to your appointment.

Asthma Annual Review

If you suffer from asthma it is important that you have a annual routine check up with one of the specialist asthma nurses in our practice to make sure that your asthma control is as good as possible. We can offer you the choice of seeing the nurse in the surgery or arranging a telephone review. If you would like to see a nurse in the surgery please contact reception to make an appointment for the asthma clinic. If you are not having any symptoms from your asthma and would prefer to speak to a nurse please ask reception to arrange a telephone appointment for an asthma review. You will be given an appointment time when the nurse will phone you.

Once the nurse has reviewed the answers given in your Asthma Annual Review Questionnaire you will be sent (likely via text message link if you have a mobile number on our system) a copy of your completed Asthma Action Plan. Please see below an example copy of an incomplete Asthma Action Plan for information.

COPD

If you suffer from COPD you shall be invited for an annual COPD review at the surgery with our respiratory nurses.

If the surgery has contacted you to book a 60 minute spirometry appointment please ensure you –

  1. Wear loose clothing
  2. Eat a light meal or lunch
  3. Come with an empty bladder.
  4. No smoking for at least 4 hours before attending
  5. If you have had a chest infection and/or have been treated with antibiotics please allow a six week period to fully recover before booking your spirometry appointment.

Feno

Our respiratory nurses and GPs may recommend you to have a FENO Test. Information about what this is below.

Physiotherapy

Physiotherapy clinics run throughout the week at both sites.  Appointments are via referral by your GP. Once your GP has referred you your name will be added to the physiotherapy waiting list and you will be contacted to book an appointment. The waiting time is generally between 7 and 10 weeks.

There are currently 6 physiotherapists associated with the surgery who offer clinics.

Many of the physiotherapy appointments at The Park Surgery are held in the upstairs consulting rooms so if you are unable to be seen upstairs please specify this when booking your appointment. There are limited downstairs physiotherapy appointments available at The Park Surgery on a Tuesday and Wednesday afternoon. All appointments at Broomfield are on the ground floor so you may be offered an appointment there.

There are evening physiotherapy appointments offered at The Park Surgery on a Tuesday and Wednesday however these generally become booked up in advance. Therefore, if you require an evening appointment please be aware that the waiting time may increase.

If you are unable to attend your appointment and need to cancel when possible please allow at least 24 hours’ notice so that we are able to fill the appointment slot with someone else in need.

If you fail to attend a scheduled appointment and do not contact the surgery within 2 weeks you shall be discharged from the physiotherapy service.

Isn’t the NHS supposed to be free?

The NHS provides most health care to most people free of charge but there are exceptions and services for which fees are charged.  If you are not sure please ask to see the Practice Manager who will be able to provide further information.

Do GPs have to do non-NHS work for their patients?

With certain limited exceptions GPs can refuse to carry out non-NHS work on behalf of their patients. Whilst GPs will  always attempt to assist their patients they are not required to do Non-NHS work.

NHS Health Checks

NHS Health Checks are being offered to people aged between 40 and 74 once every five years. The check is to assess your risk of developing heart disease, stroke, kidney disease or diabetes. If there are any warning signs, then together we can do something about it. By taking early action, you can improve your health and prevent the onset of these conditions. There is good evidence for this.

The NHS health check appointments are 20 minutes and the check is based on straightforward questions and also be a simple blood test to measure your cholesterol level.Following the check you will receive free personalised advice about what you can do to stay healthy.

NHS health check appointments are available at The Park Surgery and Herne & Broomfield Surgery and are run by Nurse Justine Jones, Nurse Lorraine Rose and Health Care Assistant Sharon Northrop.

NHS health check appointments are offered by invitation only. If you are eligible and due an NHS health check you shall receive a letter from the surgery asking you to ring and arrange an appointment. Unfortunately we are not able to book you an appointment until you have received this letter.

If you have been invited but do not wish to attend for an NHS health check please complete the below form and return it to reception.

Minor Surgery

This is performed in our minor operations room both under local anaesthetic and by modern cryosurgery techniques. If you have any skin lumps or blemishes that worry you, please consult your doctor.

The cryosurgery and minor operation clinics are run by Dr Hugh Matthews.

If you are recommended cryosurgery your name will be added to the waiting list and you will be contacted by the surgery to book your appointment. The waiting time for these appointments is approximately 3 months. Cryosurgery clinics are run once a month on a Wednesday afternoon and evening at The Park Surgery.

If you are recommended a minor operation appointment please contact reception to arrange this. Minor operation clinics are run on Tuesday afternoons’ at the Herne & Broomfield Surgery.

Minor Illness Clinic

Lifestyle

These offer a general check on your health including, blood pressure, heart problems, epilepsy, smoking cessation, weight and alcohol problems.

You may be invited for an annual lifestyle clinic appointment if you have a history of hypertension, epilepsy, chronic kidney disease, rheumatoid arthritis or another health condition that needs regular monitoring.  Prior to your lifestyle review you may be asked to have some blood tests and a urine test.

Appointments in the lifestyle clinic are run by Nurse Rebecca Lock, Nurse Justine Jones and Nurse Lorraine Rose and are 20 minutes long. Lifestyle appointments are available at both The Park Surgery and The Herne & Broomfield Surgery.

Cardiac Annual Reviews

If you have a history of chest pain, angina, heart surgery or have had a heart attack you will be invited for a cardiac annual review in the lifestyle clinic. This will involve a cholesterol check and the nurse will review your general health, treatment and give you any relevant information.  Prior to this review you will probably be asked to have some blood tests and a urine test.

If you do not wish to have your cardiac annual review please complete the below ‘opt-out’ form and return it to the surgery.

Signing this form will opt you out of your Cardiac Review for a period of one year. The surgery is then required to contact you to encourage you to have a cardiac review. If at this point you wish to opt-out again a new form must be completed.
