All clinic appointments must be booked. Phone 0300 123 1678 to make an appointment. No walk-in clinics are available at this time.
Monday: 8am to 7pm
Tuesday: 8am to 7pm
Wednesday: 8am to 7pm

Friday: 8am to 3pm

For all diabetics and related problems. Diabetic Appointments are available at the Park and the Herne & Broomfield Surgery. These clinics are run by Nurse Rebecca Lock and Nurse Jane McBride.
Diabetic Annual Reviews
All diabetic patients should have an annual diabetic review with a nurse. Prior to this review, you should have some blood tests and a urine test. If you believe you are due to a diabetic annual review please contact the surgery to check and book an appointment. When attending your annual review please bring your glucose-check machine and monitor book with you, if you have one.
If you do not wish to have your diabetic annual review please complete the below ‘opt-out’ form and return it to the surgery.
Diabetic review patient opt out form
Signing this form will opt you out of your Diabetic Review for a period of one year. The surgery is then required to contact you to encourage you to have a diabetic review. If at this point you wish to opt-out again a new form must be completed.
